  • We are located within the Avondale Business Center located at 6323 Avondale Ave, Chicago, IL. 60631. You will enter through the entrance with “6323” above the glass doors. Once inside, walk toward the elevator. Before reaching the elevator, turn right. We are the first door on the right- Suite 110.

  • You will be evaluated 1:1 by a Doctor of Physical Therapy. We will discuss your concerns and medical history. You will be asked questions regarding your bladder, bowel, and sexual health. An assessment will follow to evaluate your posture, movement patterns, strength and more. With consent, an assessment of your external and/or internal pelvic floor may be recommended.

  • No, the state of Illinois is a direct access state meaning you do not need a referral for physical therapy evaluation. However, some insurances might require a referral if you plan to submit for reimbursement through your insurance.

  • At this time we have chosen to be out- of-network for all insurances. This allows us to provide one-on-one individualized care without restrictions placed by insurance companies.

    You may be able to submit for “out-of- network” reimbursement with your insurance. It is the responsibility of the patient to contact their insurance to determine eligibility. Envision Pelvic Health & Wellness does not guarantee reimbursement by insurance companies.

    We are able to provide an itemized receipt with all necessary information for reimbursement at the patient’s request (with the exception of Medicare or Medicaid patients).

  • Yes! We understand the demands of being a busy parent. We would never want you to miss an appointment due to childcare restraints.

    Yes! We encourage you to bring a support person with you to your appointment. We want you to feel comfortable and supported.

  • We recommend wearing comfortable clothing and supportive footwear.

  • We want the patient to be comfortable and leave this up to the patient to decide. We do provide external and internal treatment when you’re menstruating. Should you prefer, we can opt to utilize the session to address mobility and strength through exercise.

  • This Patient Notice of Privacy describes the health information we collect and shows the ways in which your medical information can be used. You must sign that you have received the Notice of Privacy statement before you begin treatment.

    Understanding your Health Information

    Each time we work together, a record of your visit is made. Typically, this record contains your symptoms, examination and test results, diagnosis, treatment, and a plan of care for your treatment. This information is often referred to as your health or medical record.

    Your Rights

    Although your health record is the physical property of Envision Pelvic Health and Wellness, the information belongs to you. You have the right to:

    • Obtain a copy of the notice of privacy practices upon request

    • Inspect and copy your health record

    • Make changes in your health record

    • Make a list of who your medical record was shared with

    • Request communication of your medical record in certain places (for example, you may want us to call you at work instead of home)

    • Request a restriction on certain uses and sharing of your information

    • Revoke your permission for use or sharing of your medical record except to the extent that action has already been taken

    My Responsibilities

    We are required to:

    • Maintain the privacy of your medical records

    • Provide you with this notice as to our legal duties and privacy practices with respect to your medical records we collect and maintain about you

    • Abide by the terms of this notice

    • Notify you if we are unable to agree to a request restriction

    • Accommodate reasonable requests you may have to communicate your medical records by alternative locations

    We will not use or share your medical record without permission, except as described in this notice.

    Use and Sharing of Health Information for Treatment, Payment, and Health Operations

    The law allows me to use your medical records without your permission for treatment, payment, and business operations. The following are some examples: patient billing, third party billing, quality of care and improved services, other specialty care, caregiver/ family member notification/ communication, research, organ and tissue donation, marketing, fundraising, Food and Drug Administration (FDA), Workers Compensation, law enforcement, and public health.

    Federal law makes provision for your medical records to be released to an appropriate health oversight agency, public health authority or attorney, provided that a workforce member of business associate believes in good faith that we have engaged in unlawful conduct of have otherwise violated professional or clinical standards and are potentially endangering one or more patients, workers, or the public.

  • Pelvic Rehabilitation Practitioner Certification (PRPC) is a certification that distinguishes a provider as being an expert in the field of pelvic rehabilitation. Qualifications for this certification include 2,000 hours of direct patient care within pelvic health, and receive a passing score on the certification exam.
